Understanding benefit statements

A benefit statement contains a lot of information. Our key to reading it:

1. Your contact for this benefit statement
  If you have any questions about your benefit statement, simply contact your customer centre for assistance.
2. Insured person
  The name of the person to whom the benefit statement relates is shown at the very top.
3. The billing party
  This shows you who issued the bill and when the treatment took place. You can also see who has paid the bill: you or the health insurer.
  Separate cost settlement applies for each bill. If we batch-process multiple bills, you will also receive an overview.
4. The medical service
  The first column in the table shows you which benefits are covered by basic health insurance (compulsory health insurance) or a supplementary insurance policy. To the right, in the second column, you will find the amount.
5. How much do you have to pay?
  The next 3 columns show you how much you have to pay yourself and a breakdown of this amount.
  
  Your : the health costs that you pay yourself each year.
  
  The is set by law: you pay 10% of the costs – up to a maximum of CHF 700.–. Once you have reached the maximum amount, you do not have to pay any further excess that year.
  
  This column lists amounts for benefits that are not covered by basic health insurance or any of your supplementary health insurance policies.
6. Your share of the bill
  If we have paid the bill, this shows the share that you still have to pay yourself. We will invoice you for this amount.
  If you have already paid the bill yourself, you will see the share that we cover. We will refund you this amount.
7. Status of your co-payment
  You must bear the medical costs yourself until the annual deductible is reached. You will then contribute to the costs with a 10% excess.
